The Joint Replacement Podcast – Episode #33
Modern hip and knee replacement has changed dramatically over the last two decades. Today’s implants last longer, surgeries are more precise, recoveries are faster, and technology continues to evolve at an incredible pace. But with so much information available online, patients often wonder: When is it actually time for surgery? How long will my implant last? Is robotic surgery really better?
To answer these questions, I sat down with Dr. Ran Schwarzkopf, Professor of Orthopedic Surgery, Associate Chief of Adult Reconstruction, and System Director of Strategic Initiatives and Operations at NYU Langone Health. Dr. Schwarzkopf is internationally recognized for his expertise in primary and revision hip and knee replacement, robotic-assisted surgery, implant innovation, and surgical education.
Our conversation explored not only where joint replacement is today, but where it is headed over the next decade.
How Robotic Surgery Has Changed Joint Replacement
One of the major themes of our discussion was the role of robotic-assisted surgery.
Dr. Schwarzkopf emphasized an important distinction that often gets lost in public discussions. Today’s robotic systems are not autonomous robots performing surgery on their own. Instead, they function as highly sophisticated surgical instruments that allow surgeons to execute a carefully planned operation with greater precision.
Rather than replacing the surgeon, robotics enhances accuracy by helping surgeons:
- Position implants more precisely
- Measure soft tissue balance throughout surgery
- Execute individualized surgical plans
- Reduce technical variability
The surgeon remains in complete control throughout the procedure.
Hip Replacement and Knee Replacement Are Very Different Operations
One of my favorite analogies from our discussion compared hips and knees to vision correction.
Dr. Schwarzkopf describes:
- Hip replacement as contact lenses
- Knee replacement as eyeglasses
A well-performed hip replacement often becomes something patients barely think about once they’ve recovered.
Knee replacement, however, is inherently more complex.
Unlike the hip, the knee contains multiple ligaments, tendons, muscles, and moving surfaces that all interact during every step. Even with modern implants, recreating a perfectly natural-feeling knee remains one of the greatest challenges in orthopedic surgery.
Fortunately, technologies such as robotics and intraoperative balancing are helping surgeons move closer to that goal.
How Long Do Modern Joint Replacements Last?
Many patients still arrive in clinic believing that joint replacements only last 10–15 years.
That advice was reasonable decades ago.
Today, it is increasingly outdated.
One of the biggest advances has been the development of highly cross-linked polyethylene, the plastic bearing surface used in modern hip and knee replacements.
This material has dramatically reduced implant wear and has allowed surgeons to use larger femoral heads in hip replacement, improving stability while maintaining excellent durability.
For many patients over age 50, modern implant materials may very well last the remainder of their lifetime.
Are Patients Ever Too Young for Joint Replacement?
Another important topic involved younger patients.
Many people assume surgeons avoid replacing joints in patients under 50.
Dr. Schwarzkopf challenged that misconception.
The decision should not be based on age alone.
Instead, it depends on:
- Severity of arthritis
- Quality of life
- Ability to work
- Daily function
- Expected benefits versus future revision risk
For younger patients with severe arthritis, restoring decades of pain-free function may outweigh concerns about requiring another surgery later in life.
When Is the Right Time for Surgery?
Patients frequently ask:
“Should I wait?”
Dr. Schwarzkopf offered one of the most practical ways I’ve heard to answer this question.
He asks patients:
“If your pain stayed exactly where it is today for the next five to ten years, would you be satisfied?”
If the answer is yes, waiting often makes sense.
If the answer is no—and arthritis is preventing patients from living the life they want—joint replacement becomes a much more reasonable option.
Joint replacement is ultimately a quality-of-life procedure.
The timing depends less on the X-ray and more on how arthritis affects everyday living.
Revision Surgery: Solving Complex Problems
Our conversation also explored revision joint replacement.
Unlike primary joint replacement, every revision case presents unique challenges.
Revision surgeons frequently treat:
- Failed implants
- Infection
- Bone loss
- Instability
- Fractures
- Multiple previous surgeries
These procedures require extensive planning, specialized implants, and individualized surgical strategies.
For surgeons who enjoy complex problem-solving, revision arthroplasty remains one of the most rewarding areas within orthopedics.
What Makes an Exceptional Surgeon?
One of the most interesting discussions centered around surgical training.
Dr. Schwarzkopf believes technical excellence begins long before entering the operating room.
He discussed qualities such as:
- Three-dimensional spatial awareness
- Pattern recognition
- Deliberate practice
- Learning from complications
- Constant refinement of technique
- Visiting other surgeons to continue improving
One quote stood out:
“The goal is never to be fast. The goal is to be efficient.”
Experience teaches surgeons which parts of an operation require patience and where precision matters most.
The Future of Joint Replacement
Looking ahead, Dr. Schwarzkopf believes several exciting developments are on the horizon.
These include:
- Personalized robotic surgical planning
- Better understanding of individual knee alignment
- New implant designs made possible by robotic bone preparation
- Continued improvements in implant materials
- Artificial intelligence assisting with surgical planning and data analysis
Rather than replacing surgeons, these technologies will likely help surgeons deliver increasingly personalized care.
Final Thoughts
This conversation reinforced something I strongly believe:
Technology alone does not produce better outcomes.
Instead, the combination of experienced surgeons, thoughtful decision-making, modern implants, robotics, evidence-based perioperative care, and realistic patient expectations continues to improve joint replacement for patients around the world.
